For more than 40 years, Sitrin's daytime adult program has been a popular spot for individuals in the Mohawk Valley. This innovative program offers the best of both worlds, providing assistance to those who need it, but do not require skilled nursing care, and opportunities for social activities and engagements.

Located within Sitrin Health Care Center on Tilden Avenue in New Hartford, this program provides flexible, individualized services during the day, making it possible for individuals to continue living at home while receiving the personal care, nutrition, and therapies they need at Sitrin.
Just as importantly, the program gives adults the opportunity to make friends, take part in activities, and enjoy life in a safe and supportive environment – one that brings a greater sense of well-being to participants and their families.
Selection of Services
Sitrin's Adult Day Health Care program offers a comprehensive array of services that enable adults to live as full a life as possible. The program is designed to meet both the medical and social needs of adults.
Medical Model Program
In addition to having a Licensed Nurse within the program, this model is available Monday through Friday and includes meals and snacks, personal care assistance, medication administration, physician prescribed treatments, and therapy services, as well as recreational activities and community outings.
This model is specifically designed for individuals who may have physical or cognitive conditions that require supervision and monitoring.
Social Model Program
The social program is also available Monday through Friday and includes meals and snacks, cooking workshops, creative activities, lunch and community outings, and theme celebrations. This model provides individuals with an opportunity for social interaction with peers.
Eligibility
This program is supported by Oneida County Office for the Aging and Continuing Care, New York State Office for the Aging, and the Federal Administration on Community Living.
Medical Model participants:
- require physician approval prior to registering
- must maintain attendance at a minimum of one day per week
- must remain under the care of their primary care physician
Social Model participants:
- can refer themselves to the program, or can be referred by a family member, community case manager, or Managed Medicaid provider
Payment
Sitrin's Adult Day Health Care program accepts Medicaid, private pay, veteran's benefits, long-term care insurance, Traumatic Brain Injury (TBI) Waiver program, Nursing Home Transition and Diversion (NHTD) Medicaid Waiver programs, Medicaid Managed Care programs and Office of the Aging.
