Full Range of Care Services, All in One Place
Sitrin's long-term care homes are part of the same care community as our medical rehabilitation facility, where we are able to provide our residents with a broad range of therapy services, thereby minimizing the need for hospital care. This results in fewer transfers to other health care facilities, which means less disruption of daily life, as well as a higher degree of comfort for our residents and peace of mind for their families.
Long-Term Care Residences
Sitrin's unique long-term care homes are a dramatic departure from the traditional nursing home model, featuring:
- Nine houses (eight accommodate 12 people, one accommodates 14)
- Each home includes dining area, living room, and fully equipped kitchen
- Private bedroom and bathroom for each resident
- Gardens and beautifully landscaped yards
- Community Center for events, activities, and gatherings
Private pay rate for room and board for long-term care residences is $420/day. There is a 6.8% New York State assessment fee, in addition to the room and board rate.

The New York Association on Independent Living Open Doors Program
The New York Association on Independent Living (NYAIL) Open Doors program is available to assist nursing home residents interested in returning to community life and gaining access to necessary services. Open Doors can be contacted by referring to the regional listing identified at https://ilny.us/transition-assistance, by calling NYAIL's Transition Center Hotline at 844-545-7108, or via email to secq@ilny.org. One of the programs available to support individuals in the community is the Nursing Home Transition and Diversion (NHTD) Waiver Program. Referrals to the NHTD Program can be made by contacting the Regional Resource Development Center (RRDC) or by contacting the Open Doors Program.
